Public long-term care systems never cover the whole of later life. Families mix insured care with extra services: meals, shopping, extra hours, education and household help.
Common extras
- Meal delivery and help with shopping so nutrition does not depend on one exhausted caregiver
- Additional home-care hours outside the insured plan
- Short stays when a caregiver is ill or travelling
- Transport to clinics, adult day care and family events
- Home modifications: lighting, rails, bathroom safety
- Education for family caregivers and paid staff
In Japan these were often labelled 保険外サービス — useful, but easy to confuse with insured デイサービス. Ask what is billed to 介護保険 and what is private. Elsewhere the same mix appears as “private duty” plus Medicare/Medicaid home health.
